By signing below, Player and Guardian acknowledge that they have read, understood, and voluntarily agree to the terms of this Agreement.
JSA Ascend Camp
Service Agreement & Liability Waiver
This Service Agreement & Liability Waiver (“Agreement”) is entered into by the undersigned participant (“Player”) and their parent or legal guardian (if Player is under 18) (“Guardian”), in consideration for participation in JSA training camps, programs, events, and related activities operated by JSA (“JSA,” “we,” “us,” or “our”).
By registering for or participating in any JSA program, the Player and Guardian acknowledge, understand, and agree to the terms below.
1. Acknowledgment of Risk
Player and Guardian understand that participation in soccer training, fitness activities, and related athletic programming involves inherent risks, including but not limited to:

Sprains, strains, fractures, or other musculoskeletal injuries

Concussions or head injuries
-
Heat-related illness or dehydration
-
Collisions with other players, equipment, or surfaces

Aggravation of pre-existing medical conditions
Player voluntarily chooses to participate in JSA programming with full knowledge of these risks.
2. Assumption of Risk
Player and Guardian knowingly and voluntarily assume all risks associated with participation in JSA programs, whether known or unknown, foreseeable or unforeseeable, including risks arising from negligence on the part of JSA, its coaches, staff, or representatives, to the fullest extent permitted by law.
3. Release of Liability
To the fullest extent permitted by law, Player and Guardian hereby release, waive, and discharge JSA, its owners, coaches, staff, contractors, and affiliates from any and all claims, demands, causes of action, or liability for injury, illness, death, or property damage arising out of or related to participation in JSA programs.
This includes, without limitation, claims arising from ordinary negligence.
4. Medical Treatment Authorization
In the event of injury or medical emergency, Player and Guardian authorize JSA staff to seek medical treatment on behalf of the Player if Guardian is not immediately available. Player and Guardian agree to be responsible for any and all medical expenses incurred.
5. Fitness to Participate
Player and Guardian certify that the Player is physically and medically fit to participate in high-intensity athletic training. It is the responsibility of the Player and Guardian to consult with a physician prior to participation if there are any concerns.
6. Code of Conduct
Player agrees to uphold a standard of respect, effort, and discipline while participating in JSA programming. JSA reserves the right to remove any participant for behavior deemed unsafe, disruptive, or inconsistent with program standards, without refund.
7. Confidential Information Agreement
Player and Guardian agrees to hold in confidence and not to disclose or use any confidential information, materials, or strategies provided by JSA, including but not limited to coaching contacts, proprietary processes, and placement strategies.
8. Media Release
Player and Guardian grant JSA permission to capture and use photos, video, and other media of the Player during training sessions for promotional, educational, and marketing purposes, unless otherwise requested in writing.
9. Governing Law
This Agreement shall be governed by and interpreted in accordance with the laws of the State in which JSA operates its programming, without regard to conflict of law principles.
10. Acknowledgment
By signing below, Player and Guardian acknowledge that they have read, understood, and voluntarily agree to the terms of this Agreement.
